WHAT WE DO

Delaware Valley Community Health, Inc. employs highly skilled, certified and accredited healthcare professionals who are culturally sensitive, friendly and respectful. Our goal is to provide you with the best healthcare possible and keep you informed and educated every step of the way. Clinicians and clinical support staff offer a full range of services, including Adult & Family Medicine, Pediatrics, Women’s Health Care, Behavioral Health, Dental Health, Social Work, Health Education and other ambulatory services at 8 convenient locations in Philadelphia, Montgomery, and Delaware Counties.

We accept all insurances and patients will be eligible for DVCH’s Sliding Fee Discount Scale (SFDS), according to their income and family size. The SFDS Program is intended to minimize financial barriers to care for patients at or below 200 percent of the Federal Poverty Guidelines (FPG). A Schedule of Discounts is available to all patients who qualify based on their gross household income and family size.

ADULT MEDICINE

Our Adult Medicine services are comprehensive with an emphasis on disease prevention and management. We provide age appropriate screenings, diagnosis, and treatment of diseases. Our board-certified/eligible medical staff are comprised of Physicians, Physician Assistants and Nurse Practitioners. Our providers have admitting privileges at Temple University, St. Christopher’s, and Suburban Community Hospital when hospitalization is necessary. Adult Medicine services include:

    • Walk-in visits, no appointment needed at select locations
    • Physicals: Annual (Routine), Driver’s license, School, Sports, Volunteer Services, Work Related
    • Vaccines
    • EKG Testing
    • Screenings: Blood Pressure, Diabetes, Hearing, Vision
    • Asthma Treatment
    • STD Testing
    • Coordinated Specialist Care, when necessary

BEHAVIORAL HEALTH CONSULTATION

Behavioral Health Consultation Services at DVCH are coordinated with your Primary Care Provider (PCP) to help manage your overall health. Together the Behavioral Health Consultant (BHC) and PCP consider the behavioral, physical and emotional aspects of your health to help you determine the plan that is best for you.

What is the Behavioral Health Consultation service?

We are aware that your physical health affects every aspect of your life. We also know that stress and life circumstances can affect how you feel and how you manage your health. The Behavioral Health Consultation services offer brief services that help:

  • People manage stress
  • Solve day to day life problems
  • Teach skills to improve quality of life and daily functioning, and
  • Provide necessary resources and referrals as needed.

What types of concerns does the BHC help with?

The BHC assists patients with various stressors and conditions. Here are a few reasons patients may see the BHC:

  • Family trouble
  • Stress
  • Difficult life problems
  • Medical problems
  • Loss of a loved one
  • Changes for healthy weight
  • Diabetes Management
  • Changing behaviors like tobacco or alcohol use

What should I expect when I meet with the BHC?

You can expect the BHC to ask you questions about your physical symptoms, emotional concerns, your daily actions, and how all of these might be related.

The BHC will provide brief, solution-focused treatment. You can also expect to be seen the same day as your medical appointment. The BHC and your PCP work closely together to provide your care.

Who is eligible to receive BHC services?

The BHC services are available to all patients of the health center.

How do I meet with the BHC?

Your PCP can set up a same day appointment or a BHC appointment can be scheduled at the front desk. Walk in appointments are also available.

CARE COORDINATION

The Care Coordination Department works collaboratively with the DVCH care team, patients/families, and outside contacts to facilitate quality health care services for the DVCH patient population. The Care Coordinators act as patient advocates and navigators to support better health outcomes for our patients. Their role involves clinical responsibilities, customer service, and administrative duties. Our care coordinators are trained Licensed Practical Nurses, bilingual, and have extensive case management experience.

DENTAL

COVID-19 UPDATE: Don’t delay treatment, we have numerous protocols and safety measures in place to keep you safe!

The DVCH Dental Department strives to provide comprehensive dental treatment in a friendly and professional environment. Our services include cleanings, exams, x-rays, fillings, some root canals and extractions, periodontal treatment, crowns, bridges and dentures. We also work with a network of specialists should you need a referral for treatment. Appointments are available for both children and adults. We are proud to participate with all Pennsylvania Medicaid plans and most commercial insurance plans. We accept all insurance and have a sliding fee schedule available for those who are under/uninsured. If you purchased your health insurance from the Health Insurance Marketplace (now known as Pennie), your child is eligible for one fully covered dental check-up every 6 months until the age of 19. Staff are available and happy to answer any questions you may have.

FAMILY MEDICINE

Our Family Medicine services are a combination of both Pediatric and Adult Medicine services with some members of the department also providing Gynecologic (GYN), including:

  • PAP Smears
  • Routine Gynecological (GYN) services
  • Mammography referrals
  • Birth Control counseling
  • Coordinated Specialist Care, when necessary

Our providers have admitting privileges at Temple University, St. Christopher’s, and Suburban Community Hospital when hospitalization is necessary.

GYNECOLOGY

Our women’s health services focus on the unique needs of our female patients including routine preventive care such as Pap smears, breast exams, referrals for mammography, birth control counseling and other gynecologic screenings.

Other Gynecology services include:

  • Birth control and Family Planning
  • STD testing
  • HPV testing and Gardisil Vaccine (the HPV vaccine)
  • Referrals for Colonoscopy
  • Coordinated Specialist Care, when necessary

HEALTH EDUCATION

Our Health Education services promote health and disease prevention, disability and premature death through education-driven voluntary behavior change activities. If you are a patient you can request an individual appointment with a health educator. You must be a DVCH patient to participate in nutrition and diabetes classes. Smoking cessation classes are available to the general public.

Other Health Education services include:

  • One on one and group health education-based counseling
  • Diabetes, Nutrition (related to health conditions) and Smoking cessation classes

HEALTH INSURANCE ENROLLMENT

Need Help Signing Up for Health Insurance? DVCH provides preventive care and treatment to patients of all ages, cultures, and abilities to pay. If you are uninsured or underinsured, we can help! Speak to one of our certified assisters to see what health insurance options are available for you and your family. The 2021 Open Enrollment period through Pennie (formally known as Marketplace) Begins November 1, 2020 and ends on January 15, 2021. Medicaid Enrollment is open all year around!

What is needed to apply?

  • Recent Proof of Household Income (including Spouse and working Dependents)
  • Identification Cards/ Driver License & Social Security cards
  • Legal Immigrant Documents (Green Card, Work Authorization Card Naturalization Certificate, etc.)

HIV / HEP C CARE

Hepatitis C (HCV) is now an easily treated disease and DVCH is proud to participate in Philadelphia’s goal to eliminate HCV. We have experienced clinicians that offer HCV treatment regardless of insurance status. Care Coordinators ensure that every patient has access to treatment and follow up as needed. We also offer linkage to substance abuse treatment, harm reduction and education around the risks of re-infection of HCV.

HIV SERVICES

DVCH offers comprehensive HIV services including HIV screening, preventive medicine (PrEP), treatment and monitoring. Patients living with HIV are never charged a co-pay or lab-associated fees regardless of insurance status. Care coordinators are available to assist with housing and insurance issues as well as link to medical case managers and legal services as needed.

Legal

The Medical Legal Community Partnership provides free civil legal services on site to low-income patients at local community health centers. Legal advocates, working in concert with healthcare providers, can holistically address social determinants of health that can sometimes make patients’ lives chaotic: domestic violence, unsafe living conditions, child custody, debt, evictions, mortgage foreclosure, loss of benefits, etc.

MEDICATION ASSISTED TREATMENT

The DVCH Medication-Assisted-Treatment (MAT) clinic is based out of our Fairmount Primary Care at Girard office. We recognize substance use disorder as a chronic medical condition and practice a primary care-based approach. We strive to treat the whole patient, and have an interdisciplinary team consisting of clinicians, behavioral health consultants, a clinic coordinator, a certified recovery specialist, a medical-legal partnership, and a dedicated support staff. Our goal is to improve the health and well-being of our patients by treating their substance abuse disorder, as well their other chronic medical problems and promoting disease prevention and screening. We specifically use buprenorphine and naltrexone-based therapies to treat opiate and alcohol addiction and offer supportive primary care for other substance use disorder.

MEDICATION DISPENSARY

Three DVCH sites offer an on-site Physician Medication Dispensary. Most medications that you would be prescribed during your visit are available at the Dispensary. We utilize the 340B Drug Discount Program to obtain affordable medications for those who are uninsured. For all other patients, we accept most insurance plans.

SERVICE AVAILABLE AT THESE LOCATIONS:

The Optometry department offers comprehensive services for all conditions of the eye by licensed Doctors of Optometry. A proper prescription for eyeglasses can significantly improve your quality of life and day-to-day activities, and routine eye examinations can help detect early problems with the eye and ensure healthy eye development. Comprehensive eye examinations are especially important for children because many eye conditions, such as “lazy eye,” can go undetected until later in life, when they are much more serious. If you purchased your health insurance from the Health Insurance Marketplace (now known as Pennie), your child is eligible for one fully covered eye examination and one pair of eyeglasses per calendar year until the age of 19. We accept most forms of health insurance and offer a Sliding Fee Discount Program (SDFS) rate for those who are underinsured and uninsured. If you are a patient at any of DVCH’s Health Centers, you may use the Optometry services located at Norristown Regional Health Center.

Adult and Pediatric Primary Vision Care Services include:

  • Eye Examinations
  • Assessment of Eye Health with Pupil Dilation
  • Testing and Treatment for Vision Problems
  • Eyeglass Prescriptions
  • Optical Coherence Tomography (OCT)
  • Visual Field
  • Fundus Photography
  • Auto-refraction

PEDIATRICS

Our Pediatric practices provide primary medical care for children and adolescents from birth through age 17. Services are provided by a team of  Physicians, Physician Assistants and Nurse Practitioners. Our providers have admitting privileges at Temple University, St. Christopher’s, and Suburban Community Hospital when hospitalization is necessary.

Well-Child services include:

  • Immunizations
  • Screenings: Developmental, Hearing, Vision
  • Physicals: Annual (Routine), Driver’s license, School, Sports
  • Blood Work
  • Spirometry testing
  • Coordinated Specialist Care, when necessary

Sick -Child services include:

  • Same day/next day appointment
  • Private waiting area
  • Immediate triage upon arrival
  • Medical letters for school
  • Sick care only (no routine physicals provided)

PODIATRY/FOOT CARE

Our specialty foot care services diagnose and treat conditions of the foot, ankle and related parts of the leg.  Services are provided by Doctor of Podiatric Medicine (DPM). Diabetic patients are more vulnerable to foot problems because diabetes can damage your nerves and reduce blood flow to your feet.Taking care of your feet can prevent more serious health problems.

If you are a patient at any of DVCH’s Health Centers, you may use the Podiatry Services located at Maria de los Santos Health Center and Parkview Health Center.

Other Podiatry/Foot Care services include:

  • Callus removal
  • Ingrown nails
  • Injections

PRENATAL CARE

Our Prenatal services care for the pregnant mother and her developing baby.  Practicing good nutrition, getting plenty of rest, exercising sensibly, and avoiding things that could harm mom or baby is what our team of Physicians, Physician Assistants and Nurse Practitioners help moms achieve during pregnancy.

Other Prenatal services include:

  • Prenatal Screening tests
  • Prenatal Diagnostic tests

SOCIAL SERVICES

Social services staff assess patient and family needs, identifies resources to meet those needs, educates patients regarding community resources, and assists with information and referrals to city, state and federal programs including:

  • Entitlements: cash, food, medical forms for insurance
  • Housing
  • Transportation to Specialists appointments
  • Early intervention and mental health referrals
  • Medical necessity letters to public housing, PGW and PECO
  • Durable medical equipment companies
